Social services—templates

The social services category standard suite of going out to market templates may be used by any Queensland Government agency.

Going out to market

The Request for Quote (RFQ) form (DOCX, 100.7 KB) and the RFQ Supplier Response form (DOCX, 81.9 KB) may be used when an RFQ process is used to engage a supplier.

Choosing a social services contract template

If you need to get a third party to deliver social services you should use one of the two standard contract templates.

To decide which contract template you need , you should:

  • use the Value Risk Matrix to assess the values and risks of each arrangement
  • look up any business rules that your agency might have
  • consult your agency procurement advisors or your legal unit about aspects you're not sure about.

Contract templates

There are 2 agreements used:

  • service agreement—for the majority of service delivery funding
  • short form agreement—for services or activities that are low risk and generally for funding under $100,000 per annum.

Service agreement

The Service agreement is used for the majority of service delivery funding and is made up of 3 parts:

  1. Service agreement—standard terms (PDF, 558.5 KB)
  2. Funding and Service Details (FSD), which
    • align with the whole of government Service Agreement—Standard terms
    • include 1 or more Funding Schedules
  3. Funding Schedules (FS), which
    • form part of the Funding and Service Details (FSD)
    • prescribe the funding and services to be delivered and reporting requirements.

Short form agreement

We use short form agreements for low risk and low value services or activities. The agreement is made up of 2 parts:

  1. Short Form terms and conditions (PDF, 319.5 KB)
  2. Particulars in 2 versions: (refer to your department 's Particulars template)

Where an organisation has a service agreement in place, funding may be added to the existing service agreement through a funding schedule, rather than entering into a separate short form agreement.

Supporting documents and special conditions

Additional contractual documents or special conditions may be needed to support these templates:

  • Service Agreement—Standard Terms: requires Funding and Service Details and Funding Schedules
  • Short Form—Terms and Conditions: requires Particulars and Funding Schedules, or Letters of Agreement.

If something is particularly high-risk, high-value or has any risks that are not covered by the template conditions, you will also need to include special conditions within the Funding and Service Details. If you need help to develop special conditions, talk to your Procurement or Legal Services team.

Variation templates

A variation is a change to a contract. A variation could include a change to the service level, delivery, timeframe or funding.

Deed of Variation (DOCX, 61.7 KB) : varying a contract allows for a change to certain terms or obligations without redrafting the entire contract

Deed of Novation (DOCX, 70.3 KB) : may be used when a party seeks to transfer its rights and obligations under an existing contractual arrangement to another party
