Complete this form if you’re an accumulation member and want to update your voluntary after-tax contribution.

Before you begin

For before-tax contributions to your super by salary sacrificing, contact your salary packaging provider or refer to the Queensland Government’s Salary packaging administration standing offer arrangements.

If you’re a defined benefit member, use the defined benefit member form available on our Superannuation page. Do not use this form.

After you submit

You’ll receive an email with your submission attached. Submit this attachment to your payroll provider; not to your super fund.

If your agency uses Queensland Shared Services (QSS) for payroll services, submit your request through the QSS Self service centre.
